Job Title: Certified Home Health Aide (Per Diem)
Position Summary
The Certified Home Health Aide provides personal care services to patients in their homes under the supervision of a Registered Nurse or Therapist. The aide supports patient comfort, safety, and daily needs while observing and reporting any changes in condition.

We are hiring Home Health Aides that can service any, or all of these Communities.
Dallas, Mesquite, Garland, Rockwall, Sunnyvale, and Forney
Key Responsibilities
Assist patients with personal care, including bathing, grooming, dressing, toileting, and skin care
Help with light housekeeping related to patient care (changing linens, laundry, keeping area clean)
Plan and prepare simple, nutritious meals and assist with feeding as needed
Take and record vital signs as ordered (after demonstrated competency)
Assist with walking, exercises, and range of motion activities per plan of care
Assist patients with self-administration of medications
Follow safety procedures and use equipment properly
Observe, document, and report changes in patient condition to the nurse
Follow agency policies, care plans, and documentation standards
Qualifications
Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) in good standing
Meets state and federal Home Health Aide training and competency requirements
At least 18 years of age
Ability to read, write, and document care provided
Able to work independently with minimal supervision
Compassionate, patient-centered, and professional
Current BSL/CPR Certification
Valid driver’s license, reliable transportation, and current auto insurance
