Job Title: Licensed Vocational Nurse (Per Diem)
Position Summary
The Licensed Vocational Nurse (LVN) provides direct patient care in the home setting under the supervision of a Registered Nurse (RN). The LVN implements the established plan of care, performs assigned skilled nursing tasks and treatments, and monitors patient responses. This role includes reinforcing patient and caregiver education, reporting changes in patient condition to the supervising RN, and collaborating with the interdisciplinary care team to promote safe, effective care and positive patient outcomes.

Key Responsibilities
Provide direct patient care in accordance with the state Nurse Practice Act
Implement and follow the plan of care developed by the Registered Nurse
Accurately document patient care and observations in a timely manner
Educate patients and caregivers on diagnoses, treatments, and self-care
Assist the RN or physician with procedures and treatments using proper aseptic technique
Support patient learning related to self-care and treatment plans
Communicate patient needs and changes to the RN or Clinical Supervisor
Coordinate care needs with other disciplines (HHA, PT, OT, MSW, ST, Dietitian) as appropriate
Use medical equipment and supplies safely and efficiently
Participate in ongoing professional development
Perform other duties as assigned by the Registered Nurse
Qualifications
Graduate of an accredited Practical Nurse or Vocational Nursing program
Active LPN/LVN license in the state
Minimum 2 years of nursing experience (home health, community, or med/surg preferred)
Strong verbal, written, and organizational skills
Current BSL/CPR Certification
Valid driver’s license, reliable transportation, and current auto insurance
Ability to work independently while collaborating with a multidisciplinary team
